Job Description

We are seeking a compassionate Community Support Worker to assist adults with daily living and independent living skills. Responsibilities include providing practical and emotional support, developing care plans, coordinating services with health professionals, monitoring progress, maintaining accurate records, and promoting community inclusion. Candidates must have a relevant certificate or diploma, experience in social care or support work, strong communication and problem-solving skills, and the ability to work independently and as part of a team.
