Job Description
We are hiring a Home Health Supervisor – LPN to lead clinical teams providing in-home care. You will supervise LPNs and CNAs, coordinate patient assessments and individualized care plans, ensure regulatory compliance, train staff, oversee scheduling and documentation, and communicate with families and physicians. Must hold an active LPN license, have home health experience, leadership skills, strong clinical judgment, and excellent communication. Competitive salary and benefits.
