Job Description
Job Summary: The Clinical Case Coordinator is a vital member of the interprofessional team at Kinark Child & Family Services. This role involves providing case management services to youth admitted to the SAYC facility, offering individual and group psychotherapy, and coordinating resources for youth and their families throughout their admission.
Duties & Responsibilities:
- Manage services for youth and their families.
- Develop discharge plans with stakeholders.
- Conduct individual and group psychotherapy.
- Facilitate the intake process.
Qualifications: BSW required; eligible for OCSWSSW registration; excellent communication skills; 1-5 years of relevant experience.
